Position Summary

The Promise Senior Services Community Liaison is a frontline staff who works with individuals from diverse backgrounds and has a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison between social services and the community to facilitate access to resources and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing knowledge and self-sufficiency through a range of culturally and linguistically appropriate activities and services such as outreach, community education, and advocacy. This position is designed to work with SSL residents 55+ years of age.

Essential Responsibilities And Duties

  • Community Outreach & Engagement
  • Assess needs and facilitate connections to services, programs, and resources for seniors, including those who may be unserved or underserved.
  • Work with seniors, senior housing and care facilities to creatively solve obstacles to accessing resources such as health care, food security, education, and employment.
  • Provide mental health resources and information at community events and activities.
  • Conduct home, office, and public location visits to assist seniors and their families, ensuring equitable access to outreach programming.
  • Monitor changes in senior clients' situations and provide follow-up assistance as necessary.
  • Assist clients in applying for financial support and refer them to appropriate sources for additional services.
  • Promote, and attend community engagement events, such as PSSL quarterly family nights and citywide events.
  • Recruit senior residents to participate in a variety of community councils, coalitions, and focus groups.
  • Identify ways to connect or build a social infrastructure to support SSL seniors.
  • Collaboration
  • Act as a facilitator between seniors and community organizations.
  • Build and maintain relationships with community partners, senior centers, and community agencies.
  • Provide short-term counseling and act as a liaison between various community service providers.
  • Interact regularly with the Director and office staff for specific case management, to share concerns, and to arrange appropriate additional services.
  • Participate in multiple cross-functional groups, committees, and boards to support the senior population.
  • Work with a supervisor to identify and expand partnerships that serve seniors in the SSL community.
  • Work collaboratively with the PSSL Community and Family Engagement Team and partners to align efforts and streamline services. (Housing, Food, Digital Access, and others)
  • Recruit and train volunteers to work with SSL senior residents.
  • Recruit and train SSL senior residents to work with PSSL youth programs.
  • Administration
  • Supports community center and participant needs
    • Opening and closing, staffing the front desk, maintaining clean and organized spaces
    • Supports the overall safety, order, and accountability of participants and staff
    • Assists participants in accessing programs, resources, classes, or services (printing, devices, etc.).
  • Maintain logs from home visits and other outreach activities through Unite US tracking tool.
  • Maintains confidential client files and inputs all data and interactions in the Unite US database.
  • Take notes and provide reports/updates from meetings.
  • Gather resource materials for distribution at community events and sites.
  • Able to differentiate between basic client needs and those of a more serious nature. Can identify an emergency, call 911, and inform the Director.
  • Attends relevant seminars and training programs to maintain knowledge of elder service programs and support service delivery systems.
  • Participate in the South Salt Lake Cares Coalition (Education, Health, Arts, Neighborhood, Jobs and Economy, Safety, Housing, Interfaith, and Social Impact) and supervise City events as assigned.
