Social Services Coordinator (Social Worker Clinical)

Job Summary:

The Social Services Coordinator (Clinical Social Worker) provides clinical and administrative support to patients and families by assessing psychosocial needs, coordinating care, and connecting them with community and healthcare resources. This role ensures compliance with regulatory standards while promoting patient-centered care and overall well-being.

Key Responsibilities:

Conducts psychosocial assessments and develops individualized care plans.

Provides clinical counseling and crisis intervention for patients and families.

Coordinates referrals to healthcare providers, community agencies, and social service programs.

Advocates for patients’ rights and access to resources.

Documents assessments, interventions, and progress in compliance with standards.

Collaborates with interdisciplinary teams to support holistic patient care.

Supports discharge planning and care transitions.

Participates in quality improvement initiatives and staff education.

Qualifications:

Required:

Master’s degree in Social Work (MSW) from an accredited institution.

Current state licensure as a Clinical Social Worker (LCSW or equivalent).

Minimum 2 years of experience in a healthcare or clinical setting.

Strong knowledge of psychosocial assessments, crisis intervention, and community resources.

Excellent communication, organizational, and documentation skills.

Preferred:

Experience in hospital, ambulatory care, or long-term care environments.

Familiarity with case management and care coordination systems.

Bilingual abilities a plus.
