FT - Monday through Friday

Knoxville Center for Behavioral Medicine - a comprehensive inpatient behavioral health facility. Our center will provide acute behavioral health care and outpatient treatment to adults and geriatric adults who are in crisis from severe symptoms related to behavioral health and/or substance use disorder.

Are you a passionate about maintaining a clean and organized environment? If so, we'd love you to join our team as a Case Manager / Discharge Planner at Knoxville Center for Behavioral Health. We would love for you the team for our new hospital and carry out our vision of bringing communities, healthcare providers, and families together to improve behavioral health, one patient at a time. We provide psychiatric care to adults and geriatric patients who require inpatient hospitalization due to mental disorders like cognitive illnesses.

What You Will Be Doing

  • Monitor and manage client/patient/resident's cases through development and implementation of individual care plans, collecting clinical data and remain informed by continuous review of documentation.
  • Coordinate care by tracking, monitoring, and reporting on inpatient or outpatient care.
  • Manage client/patient/resident's cases with knowledge and expertise with the patient population of the facility.
  • Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist, or psychiatrist.
  • Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
  • Act as a liaison between referral sources, patients, and counselors.
  • Ensure all patient discharge planning documentation is clear, concise, and timely.

What You Will Need

  • High school diploma required.
  • Bachelor's degree in Health Administration, Social Science or a related field or relevant previous job experience preferred.
  • Ability to create and maintain professional liaison with community agencies and organizations to ensure continuity of care.
  • Knowledge of mental health and substance abuse fields is preferred.
