As a Hospice Social Worker, here’s a big-picture view of what you’ll do:
- Provide emotional and social support for patients facing terminal illness and their families
- Assist families in coping with grief, anticipatory loss, and stress related to caregiving
- Identify psychosocial needs and connect patients and families to community resources
- Conduct psychosocial assessments and contribute to the patient’s plan of care with the interdisciplinary hospice team
- Provide counseling, crisis intervention, end-of-life planning, and bereavement support
- Advocate for patients’ wishes while helping families navigate healthcare and community systems
- Collaborate with RNs, chaplains, hospice aides, physicians, volunteers, and therapists, ensuring holistic care for patients and families
A few must-haves for a Hospice Social Worker (MSW):
- Master’s degree in Social Work and active license in Illinois
- Minimum of 1 year of social work experience (hospice or healthcare preferred)
- CPR certification
- Knowledge of hospice regulations, psychosocial dynamics of death and dying, and available community resources
- Strong interpersonal skills, excellent communicator, empathetic, compassionate, and resourceful
