As a Hospice Social Worker, here’s a big-picture view of what you’ll do:

• Provide emotional and social support for patients facing terminal illness and their families
• Assist families in coping with grief, anticipatory loss, and stress related to caregiving
• Identify psychosocial needs and connect patients and families to community resources
• Conduct psychosocial assessments and contribute to the patient’s plan of care with the interdisciplinary hospice team
• Provide counseling, crisis intervention, end-of-life planning, and bereavement support
• Advocate for patients’ wishes while helping families navigate healthcare and community systems
• Collaborate with RNs, chaplains, hospice aides, physicians, volunteers, and therapists, ensuring holistic care for patients and families

A few must-haves for a Hospice Social Worker (MSW):

• Master’s degree in Social Work and active license in Illinois
• Minimum of 1 year of social work experience (hospice or healthcare preferred)
• CPR certification
• Knowledge of hospice regulations, psychosocial dynamics of death and dying, and available community resources
• Strong interpersonal skills, excellent communicator, empathetic, compassionate, and resourceful
