Opportunity: Pediatric Medical Home Care Coordinator (MHCC)
We are excited to welcome a new Pediatric Medical Home Care Coordinator (MHCC) to our close-knit team. This will be a part-time role, with the potential of growing into a full-time position as we continue to build our patient base. This is an incredible opportunity for a self-starter with a spirit to contribute to the growth and evolution of our practice while providing exceptional care.
About Our Practice:
Walden Pond Pediatrics is a modern, integrated, solo practice dedicated to supporting children's growth from birth through adolescence. We believe the foundation of a child's health is rooted in the care provided during their early years, emphasizing preventive care and ample time for meaningful interactions during each visit. Our small but effective team is committed to providing compassionate, holistic care for all pediatric needs, from routine wellness to complex chronic conditions. Our practice consistently receives high marks from our families. We foster a collaborative atmosphere where innovation and patient well-being are paramount, and we actively contribute to community development by supporting youth activities and area schools. Walden Pond Pediatrics is a part of the Boston Children’s Primary Alliance network and fosters a close working relationship with Boston Children’s Hospital specialists and our local community hospital, Emerson Health.
Your Role: Impacting Young Lives
The Medical Home Care Coordinator enhances patient care through a diverse role as:
· Clinical educator
· Care coordinator
· Patient and family advocate
· Quality improvement facilitator
The MHCC works in a collaborative relationship with other members of the Patient Centered Medical Home (PCMH) team to:
· Improve delivery of comprehensive cost-effective patient and family-centered care
· Facilitate health maintenance, disease prevention, and chronic disease management
· Assist in clinical evaluation of medical and psychosocial needs for our patients and families
· Monitor and close clinical gaps in care and ensure smooth transitions of care
Additional responsibilities include, but are not limited to:
· Phone triage
· Medication and treatment administration
· Provision of health education and patient care follow-up
· Availability to work until 6pm at least one night/week
Qualifications, Experience, & Education:
· Preferred Three to five years of clinical experience with a pediatric population.
· Strong written and verbal communication skills required
· Strong critical thinking and analytical skills required
· Proficiency with Epic and telehealth platforms
· Ability to function independently and prioritize effectively
· Driver's license required
