Home Care Coordinator

• 1+ years of Home Health Care or Case Manager experience
• Knowledge of Medicaid waivers and the state EVV system preferred
• High attention to detail
• Exited self-starter, with the ability to lead and problem solve
• Excellent written and verbal skills and proficiency with computers
• Ability to travel to clients' homes
• Sandata EVV software system knowledge and experience is a plus. Preferred but not required!

Position responsibilities:

• Communication daily with clients, caregivers, families, and 3rd parties
• Documentation of all client and caregiver related communications
• Monitor daily schedules of clients and field staff
• Attend designated training and ongoing education opportunities
• Addressing & resolving complaints as needed
• Conduct the on-boarding and orientation of new caregivers and clients
• Making in home visits as needed to support the clients and direct care workers.
• Maintain reasonable case management load assigned.

FAQ

• Office hours are Monday through Friday 9:00am through 5:00pm, though hours will vary dependent on Client and/or business needs
• This position requires travel to and from Clients' homes and meetings within the Denver and sometimes Colorado Springs Metro-Area
• Full Time position
• Offer for pay is directly based on industry experience and knowledge.

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
