Job Description

Summary:
This position is ideal for a licensed counselor committed to delivering client-centered behavioral health care. As a Counselor, you’ll lead coordinated care efforts across mental health, substance use, and social service domains. You’ll design and implement treatment plans, conduct assessments, provide individual and group therapy, and coordinate with multidisciplinary healthcare teams. Your advocacy, documentation, and clinical judgment will be central to client success and safety.

What You’ll Do:

  • Develop and implement individualized treatment plans based on client needs and team input.
  • Deliver individual, family, and group counseling using evidence-based practices.
  • Provide care coordination, linking clients to medical, housing, financial, and legal services.
  • Maintain clear, complete documentation in accordance with legal and agency standards.
  • Lead engagement strategies, safety planning, and recovery-focused interventions.
  • Collaborate with physicians, nurses, case managers, and external service providers.

Experience You’ll Need:

  • Master’s degree in Counseling, Social Work, Psychology, or related field required.
  • Active clinical license or eligibility preferred (e.g., LADC, LPC, LCSW, LMFT, LPCA, LMSW, CADC).
  • Experience with substance use, mental health, and/or co-occurring disorders.
  • Strong understanding of FQHC or integrated care models is a plus.
  • Excellent written and verbal communication and documentation skills.
