Position Overview
Reporting to the Director of Avenidas Social Work Services, the Licensed Clinical Social Worker (LCSW) is responsible for providing comprehensive and compassionate social work services to older adults, their families, and significant others, including elder care consultations, the provision of psychosocial assessments, customized interventions, family mediation and conflict resolution, proactive planning for lifestyle changes and end-of-life treatment options, and bereavement counseling. The LCSW will also review and sign off on all care plans as needed. The LCSW will serve as the official LCSW for all Avenidas Social Work Services programs. LCSW will represent Avenidas to outside groups and allies. Will be an Agency representative, educating and informing the community about available services and conducting community outreach. Duties may require evening and weekend hours occasionally.
Essential Job Functions
Direct Client Services & Clinical Care:
- Oversee the design and implementation of comprehensive, compassionate social work services for older adults, their families, and significant others.
- ·Provide expert consultations for adult children and other relatives, offering strategies and resources to navigate their loved one's physical and cognitive decline.
- Conduct comprehensive psychosocial assessments of older adults to identify needs, strengths, and challenges.
- Develop and implement individualized, compassionate intervention plans tailored to the unique needs of older adults, their families, and significant others.
- Provide a range of services, including individual and family counseling and crisis intervention.
- Facilitate family mediation and conflict resolution to address interpersonal dynamics and improve communication.
- Offer proactive planning support for lifestyle transitions, healthcare decisions, and end-of-life care options.
- Provide compassionate bereavement support to individuals and families experiencing loss.
- Provides case consultation to staff and volunteers for current or potential Avenidas members.
Program Oversight & Quality Assurance:
- Review, approve, and sign off on all client care plans to ensure accuracy, comprehensiveness, and adherence to professional standards .
- Provide guidance, mentorship, and oversight to ensure the effective and timely delivery of all aspects of social work services.
Group Facilitation & Education:
- Design, develop, and lead support groups for older adults addressing various needs and concerns.
- Design, develop, and lead support groups for caregivers, providing a supportive and educational environment.
- Act as a speaker and presenter for internal and external educational speaking engagements.
- Support external partners in the development and delivery of educational sessions and series, as opportunities arise.
- Makes presentations to various external audiences about the types of services available within Avenidas and on eldercare issues.
Collaboration & Community Engagement:
- Work closely with agency staff across departments to develop and implement new community services, including Town Halls and educational series.
- Cultivate and maintain strong internal and external partnerships for cross-referral, increasing awareness of services.
- Provide case consultation to agency staff and volunteers regarding current or potential members.
Secondary Job Functions
- Key member of the Annual Caregiver Conference planning committee.
- Mentoring, guiding, and supporting developing clinicians through their licensure journey, fostering their professional growth, and ensuring the delivery of high-quality, ethical clinical services.
- Other duties as assigned by the Director of Social Work Services. Duties may overlap with services offered under the Avenidas Social Work Services specialty programs- ECM, CHW.
Qualifications
Education and Experience.
- MSW required; LCSW required. Prior experience in working with older adults, particularly through case management services.
- A minimum of 3 years of post-licensure clinical experience in gerontology, mental health, healthcare or private practice.
- Demonstrated experience providing clinical supervision and/or mentorship to pre-licensed social workers or other mental health professionals
Knowledge, Abilities and Skills
- Knowledge of and capacity to utilize community resources; ability to assess physical and social needs of frail older adults.
- Demonstrated counseling ability; demonstrated supervisory skills; good oral and written communication skills; willingness to make judgment calls and to serve as an advisor to other Avenidas staff on client issues.
- Demonstrated ability to work independently with a high degree of successful decision making for program matters. Self-directed with the ability to work as a member of varying teams of professionals.
- Demonstrated organizational and financial responsibility skills.
- Driver’s License: CA driver's license with auto insurance.
Physical Demands, Work Environment, and Vision Requirements
- Frequently stand, walk, sit, use a computer, and handle telephone communications.
- Occasionally reach/work above shoulders, twist/bend/stoop/squat, lift/carry objects up to 10 pounds, and handle paperwork.
- The role requires travel to meetings, client sites, community events,& partner sites. Candidates must have a reliable and efficient mode of transportation to ensure timely and effective participation in these engagements.
- Work environment includes but not limited to office setting, community events, donor engagements.
- Evening and weekend hours may be required for special events.
Hours & Total Compensation Overview
This is a full-time, exempt, 40-hour-per-week position. Salary range is $94,278 - $99,516.
