The Per Diem Care Manager will play a direct role in saving the lives of our patients each day by coordinating services to ensure clients receive the help they need to fulfill their potential.
You will partner with the clinical team and other service providers to establish and maintain a therapeutic environment and healthy routine for our clients.

Requirements:

• Bachelors degree in a human service field
• Excellent verbal and written English language skills
• Experience with mental health a plus
• Valid Driver’s License
• Ability to travel up to 25% of the time

Responsibilities:

• Providing case management services.
• Creating, implementing, and monitoring the client’s plan of care.
• Coordinating crisis intervention services.
• Maintaining accurate and timely documentation of all activities and services.
• Conducting a monthly face to face visit with each client.
• Maintaining professional boundaries with clients and colleagues.
• Collaborating with local agencies and community providers to coordinate relevant services for clients.

Job Types: Full-time, Part-time, Contract

Pay: $50,000.00 - $60,000.00 per year
