About Job

As a Community Care Coordinator, you will play a crucial role in ensuring the provision of care management services for our members. You will work closely with psychiatric hospitals, crisis centers, outpatient behavioral health providers, supportive living environments, and community-based organizations to ensure our members have access to the resources and treatment they need to support their overall health. Your responsibilities will include meeting members in person within their community, coordinating care management needs with various healthcare providers, visiting members in psychiatric hospital settings, and actively participating in care management and treatment team meetings.

  • Tasks (Including but not limited to)
  • Meeting with members in person within their community to secure needed behavioral health treatment and support services.
  • Coordinating care management needs of the member with the outpatient provider, hospital, PCP and others involved in his/her care.
  • Visiting members in person when admitted to a psychiatric hospital setting to assess needs, assist with discharge plans and coordinate with doctors, nurses, social workers and discharge planners
  • Flexibility to attend multiple treatment settings same day to collect information and proactively monitor member's progress towards treatment goals/discharge.
  • Participate in Care Management/Treatment Team Meetings
  • Uphold the organization’s ethics, code of conduct and customer services standards.
  • Qualifications
  • Minimum of Bachelor’s Degree in a human services field that is an accepted area of study for Nevada Medicaid. Accepted areas of study are as followed:
  • Psychology, Sociology, Community Health Sciences, Health Ecology, Public Health, Social Work, Human Development and Family Studies, Early Childhood Education, Nursing, Speech Pathology and Anthropology.
  • Skills
  • Knowledge of treatment plans.
  • Good organizational and time management skills
  • Ability to write clear, concise documentation for recordkeeping and billing purposes
  • Strong problem solving and communications skills (verbal and written)
  • Excellent crisis management skills
  • Ability to use electronic communications and internet resources.
