Utilize Health is a care management company whose mission is to improve the health and well-being of people with neurological conditions. Our specialized care management program offers a better way to manage brain injury, stroke, and spinal cord injury diagnoses allowing for better continuity of care and reduction of avoidable high-cost services. Utilize Health's Care Managers and Nurses support members as an advocate through their healthcare journey, assisting them with physical, behavioral, functional, preventative and social determinants of health needs.

JOB OVERVIEW

The Care Manager is responsible for care management of Utilize Health members in Middle Tennessee. The Care Manager conducts care coordination visits in the community and in the member’s home setting. They assist members with navigating the healthcare system, facilitate communication and access with providers and community resources, provide patient and family education that supports self-management and reduces complications, and address social determinants of health. The Care Manager acts as a patient advocate in helping members navigate their healthcare and overall well-being.

JOB RESPONSIBILITIES

  • Serve as primary representative of Utilize Health to members and act as an advocate guiding members through their care needs.
  • Manage a member caseload through in-person, phone, and electronic means of communications and coordination.
  • Conduct in-person and phone visits with members to complete screenings and identify needs to assist members in making lifestyle adjustments, learn to self-manage their care, and anticipate complications associated with their condition.
  • In collaboration with members, develop an individualized care plan with measurable goals, objectives, and interventions to ensure needs are met.
  • Ensure timely and appropriate documentation.
  • Clearly document all member interactions in the electronic medical record platform, including care plan activities, screenings, visit notes, medical and social history, communications with or on the member’s behalf.
  • Perform care coordination activities to continually monitor members’ physical and behavioral health needs, interventions, education base, and progress of their individualized care plan.
  • Consult with the Nurse on members’ progress and revise care plan as needed.
  • Help members identify, access, and navigate community services/resources so they are better able to adopt healthy behaviors; help connect the dots in care coordination activities to ensure facilitation is seamless.
  • Coach/educate members on self-care to improve to meet both personal and clinical goals.
  • Professionally interact with members’ care providers including primary care, specialists, therapists, counselors, and office staff.
  • Schedule provider appointments on behalf of members.
  • Accompany members to their appointments when needed.
  • Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing.

OTHER RESPONSIBILITIES:

  • Responsible for attending all in-services, continuing education, and annual health requirements. Onboarding and trainings may occur at the Utilize Health offices in Nashville, TN.
  • Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor.
  • Comply with the relevant regulations concerning the privacy and security of patient’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS:

  • Bachelor’s or master’s degree in a healthcare or behavioral health program OR a bachelor’s degree combined with 2+ years’ experience working in care management and/or with patients who have a brain injury, stroke or spinal cord injury diagnosis.
  • Field-based position. Ability to travel within Middle Tennessee, primarily the southeast region of Nashville, including Antioch, Smyrna, Murfreesboro, and neighboring zip codes.
  • Preferred candidate is located within Rutherford county.
  • Care Manager will have a caseload of members and will travel to visit members in areas such as Antioch, Smyrna, Murfreesboro, and neighboring zip codes.
  • Occasional travel to Utilize Health office in Nashville, Tennessee is required.

EXPERIENCE

PREFERRED but not required:

  • Experience working in a community-based setting for at least 1 to 2 years.
  • Experience in case/care management or home health.
  • Experience working with people with neurological or other complex medical conditions.
  • Certified Special Populations Specialist (CSPS).

JOB KNOWLEDGE, SKILLS AND ABILITIES

  • Passion for working with individuals who have neurological diagnoses.
  • Demonstrated ability to provide care coordination.
  • Proficient in documenting care coordination activities in an electronic medical record.
  • Excellent oral and written communication skills.
  • Proficient in Microsoft Office Suite.
