Home Instead is seeking a compassionate and experienced Licensed Practical Nurse (LPN) or Registered Nurse (ADRN) to join our team as a Nurse Care Manager. This role is ideal for a nurse with a background in skilled nursing, assisted living, or clinical care with seniors who is ready to step into a leadership and client-facing management position.

About the Role

The Nurse Care Manager will oversee client care, ensuring our seniors receive exceptional service and support. This individual will collaborate with families, caregivers, and healthcare providers to develop and maintain personalized care plans that promote independence, dignity, and quality of life.

Responsibilities

• Oversee your Own list of clients throughout the Charleston, Dorchester and Berkley counties, while overseeeing the Client Care Department.
• Conduct client assessments and develop individualized care plans.
• Ensure regular Quality Assurance visits and follow-up with clients and families
• Provide guidance and support to caregivers, ensuring clinical excellence and client satisfaction.
• Act as a liaison between clients, families, physicians, and other healthcare professionals.
• Train and mentor caregiving staff on best practices and care protocols.
• Participate in on-call rotation to support urgent client needs.
• Track and document all care updates, visits, and communication in our software system.

Qualifications

• Current LPN or RN license in good standing.
• Minimum of 2 years of clinical nursing experience in skilled nursing, assisted living, or home health.
• Experience working directly with seniors.
• Tech-savvy: proficient in Microsoft Office, Google docs, smart phones, and able to learn care software systems.
• Passion for providing compassionate, person-centered care.
• Must pass a criminal background check and drug screening
