LCSW LMHC LMSW Transitional Care Team Leader
Bethpage, NY | Salary: $80,000/year

As the Transitional Care Team Leader, you’ll work in close coordination with the Transitional Residential Setting (TRS) team and provide clinical and administrative oversight of the Enhanced Step-Down Program’s Critical Time Intervention services and supervise a multidisciplinary team.
What You’ll Do:
- Ensure that people with co-occurring Mental Health (MH) and Intellectual/Developmental Disabilities (I/DD) receive evidence-based, time-limited support as they transition from inpatient and emergency psychiatric settings to community-based living.
- Ensure continuity of care, facilitate shared treatment planning, and support stabilization and permanent housing placement.
- Foster cross-system collaboration with OMH, OPWDD, hospital systems, and community providers.
What You Bring:
- Master’s Degree in social work, psychology, counseling, or related field.
- LCSW, LMHC, LMSW, or similar licensure; Licensed Practitioner of the Healing Arts (LPHA); CTI and 9.58 certifications (within 12 and 6 months, respectively).
- Minimum 3 years of supervisory experience and 3 years of post-licensure experience working with people with I/DD, Mental Health Diagnoses, and/or Substance Abuse. (Supervisory experience can be concurrent with post-licensure experience.)
- Strong leadership, communication, computer literacy, and organizational skills, with a heart for collaboration and excellence.
- Valid driver’s license and flexibility to meet program needs.
