St. Anthony Park Home is seeking a compassionate and organized Social Worker and Admissions Coordinator to join our team. This role combines social services with admissions, supporting residents and families from the first contact through their stay.

Responsibilities:

• Coordinate the admissions process including communication, paperwork, and orientation

• Provide social services and psychosocial support to residents and families

• Participate in care planning, discharge planning, and team meetings

• Ensure compliance with state and federal nursing home regulations

• Complete accurate and timely documentation

Qualifications:

• Bachelor degree in Social Work required; Minnesota licensure preferred

• Previous nursing home experience preferred

• Knowledge of care planning, admissions, and regulatory requirements

• Strong communication and organizational skills

• Compassionate, resident focused approach
