Requirements

  • Licensed Master of Social Work required (LMSW) in theState of Michigan.
  • Must have a minimum of five years post graduate experience working with children and adults with Developmental Disabilities, persons with a Traumatic Brain Injury or serious mental health issues.
  • Ability to navigate Microsoft Excel with at least basic skill level
  • Maintain ongoing training requirements pursuant to the role including but not limited to CAFAS, and all trainings required to maintain credentials of QMHP and QIDP.

Specific Responsibilities

  • Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
  • Acts as the Chairperson of the multidisciplinary team
  • Conducts self audits as well as peer audits
  • Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
  • Coordinates and completes all necessary referrals on behalf of client
  • Maintain calendar of member contacts (future, current, past).
  • Monitor all services provided on behalf of client
  • Monitor satisfaction with services
  • Provide resource development in an attempt to reduce the amount of service gaps
  • Develop and maintain written records necessary for professional practice as defined in the procedural manual
  • Provide outreach services to individuals and their families in their respective locales and/or homes as appropriate to their unit’s function
  • Attend DWIHN meetings and trainings as required
  • Provide community education services as assigned
  • Attend and present at court proceedings as required
  • Obtain all prior authorizations and applicable coordination of care
  • Participate in clinical supervision, team meetings and case consultations
  • Obtain all prior authorizations and applicable coordination of care
  • Follow PsyGenics, Inc. professional code of conduct
  • Represent PsyGenics, Inc. in the community, at DWIHN meetings, and other events as required
  • Complete targeted case management with members at least monthly per the member’s needs and the Individual Plan of Service
  • Participate in PsyGenics Member Events and provide targeted case management during the events with PsyGenics members.
  • Perform similar or related duties as assigned

This is a full-time position - Monday - Friday from 8:30AM - 5:00PM. The position is not remote. LMSW required. Salary commensurate with licensure and experience.
