JOB SUMMARY
Provides assessment, evaluations, treatment planning, psychotherapy and social services to individuals, couples, families and groups. Participates in clinical staff development, supervision and performance improvement activities as a member of-an interdisciplinary team. .,
- Completes mental status/risk assessments
- Conducts screenings and intake evaluations.
- Provides evaluations and treatment to assigned patients.
- Conducts individual, family, and group psychotherapy that is focused and goal oriented.
- Conducts crisis counseling as requested or needed.
- Documents all evaluations, treatment, and terminations of patients in a timely manner in accordance with Morris Heights Health Center's Principles of Practice and protocols, OMH, JCAHO and NYS DOH and DOE as required.
- Accurately and completely writes progress notes, initial treatment plans, comprehensive treatment plans, termination summaries and other clinical forms for treatment, managed care
/insurance companies and outside organizations pertinent to the patient's treatment.
- Consults with other behavioral health, primary care and specialty care providers as appropriate.
- Collaborates with outside organizations such as school, hospitals, higher level of care programs, and other service programs on behalf to patient care needs.
- Maintains expected levels of productivity and provides quality care as defined by regulatory agencies and MHHC.
- Assimilates core values, compassion, dignity, respect and integrity in providing services.
- Maintain professional and ethical behavior at all times.
- Participates in training in the assessment and reporting of child abuse or maltreatment, elder abuse and maltreatment, domestic violence.
- Maintains a neat, safe, clean and organized workspace.
- Takes responsibility for own professional development and learning, to provide best practices of treatment.
- Performs other related tasks as instructed.
