Job Title: Patient Care Manager
Job Summary: Primary functions are to monitor, assist, and participate in the administration of nursing care for patients of all ages in their place of residence. Coordinates care with the interdisciplinary group, patient, family and referring agency. Assists Director of Clinical Services with monitoring and analysis of QAPI issues and assists clinical staff regarding agency policies and procedures.

Job Qualifications:
Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
Licensure: Current State license as a Registered Nurse, current Driver’s License
Experience: Must have at least one year of experience as a Registered Nurse in a hospice related setting.
Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills and ability to communicate effectively. Demonstrates proven decision-making skills. Must read, write, and comprehend English.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental and Working Conditions:
Works in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call
Physical and Mental Effort:
Prolonged standing and walking required with ability to lift to 50-lbs and move patients. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines, patient/agency needs, and meet patient/family individualized psych-social needs. Ability to make quick decisions and resource acquisition.
Essential Functions:
Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs, (i.e., risk for grief, cultural and spiritual, verbal and non-verbal).
Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
Implement/develop/document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.
Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. Prioritize any needs with the members of the IDG. Provide training to other staff as needed.
Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the care giver to meet the patient’s immediate needs upon admission and throughout care.
Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
Timely submission of all required paperwork. Demonstrate commitment, professional growth and competency. Meet productivity standards. Perform on-call responsibilities and provide on-call service to patients/families as assigned Provide effective communication to patient/family, team members, and other health care professionals
Promotes the philosophy and clinical policies of the Agency, to ensure quality of patient care.
Participates in activities relevant to professional services including development of qualifications and assignment of agency personnel.
Monitors and participates in the QAPI program and participates as a member of the Interdisciplinary group (IDG), assisting in development of IDG plan.
Monitors systems and identifies problem areas to Administrator and Director of Clinical Services both verbally and through written reports.
Promotes and educates regarding concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Facilitates the use of effective interpersonal relations and communication skills by other team members to achieve desirable outcomes.

Pay: $80,000.00 - $92,750.00 per year
