Responsibilities
- Develop, implement, and monitor individualized care plans
- Coordinate caregiver schedules and match staff with clients based on needs
- Serve as the main point of contact for families, addressing questions and concerns
- Conduct regular check-ins with clients and caregivers to ensure quality of care
- Maintain accurate documentation and compliance with agency standards
- Support caregiver recruitment, training, and retention initiatives
- Collaborate with leadership to continuously improve service delivery
Qualifications
- Previous experience in care coordination, case management, or related field required
- Strong organizational and problem-solving skills
- Excellent communication and interpersonal abilities
- Compassionate, professional, and client-focused mindset
- Proficiency in Microsoft Office and scheduling software
- Valid driver’s license and reliable transportation
