- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, at least restrictive level of care
- Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team

- Undergraduate degree or equivalent experience
- Current unrestricted RN license in the state of TX
- 4+ years of experience working within the community health setting or in a healthcare role
- Intermediate Microsoft Office experience working with Microsoft Word, Excel, and Outlook (ability to create, edit, save and send documents, spreadsheets and emails)
- Valid driver's license, reliable transportation and the ability to travel up to 75% throughout the NW Houston, TX designated Service Delivery Area (SDA) and surrounding areas to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Reside in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Reside within 50 miles of the NW. Houston, TX area
- Payment driven Performance Model (PDPM) certification or ability to obtain within 30 days of employment
- Clinical experience
- Experience in case management and/or certification in case management (CCM)
- Field-based work experience
- Experience with electronic charting
- Background in managing populations with complex medical or behavioral needs
