Job Title: Home Health Companions Concierge Care Coordinator
Location: On-Site, Dallas, Texas

Reports To: Director of Nursing and Compliance
Pay Range $22 to $26 per hour
Job Summary:
The Care Coordinator is responsible for ensuring concierge Care Professional staffing coverage and for maintaining accurate internal records. Essential duties ensure high-quality service delivery through effective scheduling, proactive communication, and timely problem resolution.
This role supports the operations team by facilitating smooth daily workflow while managing care schedules, coordinating resources, tracking compliance, and providing all necessary training and supervision. The Care Coordinator serves as the primary point of contact for caregivers and support for field nursing.
Key Responsibilities:
- Promote a professional and respectful work environment, motivating caregivers to reach their full potential.
- Build and maintain caregiver schedules according to visit requirements.
- Proactively fill open shifts and manage last-minute staffing changes.
- Assign shifts based on Care Professionals' skills, certifications, availability, and geographic location using carefully maintained records while monitoring caregiver timekeeping, attendance, and schedule compliance.
- Participate in on-call scheduling rotation for after-hours coverage, as required.
- Facilitate hands-on quarterly training, care transitions, and supervisory visits demonstrating skills like transferring patients, personal hygiene care, and assisting with daily living activities.
- Provide constructive feedback and coaching to help caregivers improve their understanding of skills and responsibilities.
- Promote a professional and respectful work environment, motivating Care Professionals to reach their full potential.
- Other duties as assigned
Requirements:
- 2+ years supervisory, scheduling, staffing, or workforce coordination experience (healthcare preferred)
- Excellent written and verbal communication skills
- Strong Microsoft Office skills (Excel, Outlook, Word)
- 2+ years of previous caregiving experience
- Demonstrates proactive and dependable organization, problem-solving, and time management skills
- Ability to thrive in a high-volume and fast-changing environment
- Experience with AxisCare is a plus
Work Conditions:
- Work on-site in Dallas office
- May need to lift and move training equipment or assist in patient mobility demonstrations.
