Hospice Social Worker

Position: Hospice Social Worker

Schedule: MondayFriday, 8:00 AM 4:30 PM

Weekend On-Call: Required 4 times per year

Home Office Location: IL Hospice & Support Care

2220 S State Hwy 157, Ste 300

Glen Carbon, Illinois 62034

Coverage Area: Entire Illinois Hospice service area

Caseload: 3545 patients

Job Responsibilities:

  • Meet with patients and families to determine resource needs and make referrals
  • Assess social support needs, provide education, and plan for end-of-life care
  • Offer counseling and support for anticipatory grief
  • Obtain consents for admission and cover end-of-life visits
  • Fill in for other social workers as needed

Preferred Skills / Experience:

  • Home health experience is a bonus (not required)
  • Prior social work experience is strongly preferred
  • This is a very independent position

  • Skilled nursing services
  • Adult and pediatric hospice and supportive care
  • Rehabilitation therapy

  • Infusion treatment rooms
  • Home medical equipment
  • High-tech respiratory care

Specialty programs include adult and pediatric asthma, cardiac, diabetes, orthopedic, and wound care.

BJC Home Care serves thousands of patients across Missouri and Illinois, covering more than 25 counties, making it the largest home care network in the region and one of the largest in the country.

Hospice care is the final stage of BJC's continuum of care, offering compassionate support for patients with life-limiting illnesses. Services are provided in the comfort of the patient's home or skilled nursing facility.

Hospice staff are sensitive to the physical, psychosocial, emotional, and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers trained in:

  • Symptom management
  • Pain control
  • Counseling
  • Bereavement services

Our hospice services also include alternative therapies such as music, art, and massage therapy. We focus on palliative care, helping patients manage pain and symptoms while living with peace and dignity.



Preferred Qualifications

Role Purpose: Under the direction of a physician, assists patients and their families in their residences to adapt and plan an interdisciplinary approach to treating the patient-family system.

Responsibilities:

  • Consults with patients, families, and staff regarding goals of care, end-of-life issues, and palliative needs
  • Provides direct clinical services to patients and families
  • Plans and implements social work services in collaboration with the healthcare team
  • Collaborates with community human service agencies to meet patient and family needs


Minimum Requirements

  • Education: Master's Degree Social Work
  • Experience: No Experience
  • Supervisor Experience: No Experience
  • Licenses & Certifications: Valid Driver's License


Preferred Requirements

  • Experience: 25 years
  • Licenses & Certifications: Licensed Clinical Social Worker (LCSW)
