Now Hiring: Field Case Manager (RN, LCSW, or LCPC)

Locations:

• Kankakee County, IL

• Northside Chicago, IL

(Must live in or near one of these areas for local travel requirements)

Overview

We are looking for 2 Field Case Managers to work with elderly adults and individuals with disabilities who qualify for in-home or nursing facility care.

• Schedule: Monday – Friday, 8:00 AM – 5:00 PM CST

• Travel: 50–75% of the time visiting members in their homes (mileage reimbursement provided)

• Work Setting: Home-based in between visits

• Pay: $35.87/hour + mileage reimbursement

Job Duties

• Visit members in their homes to complete assessments

• Create care plans and connect members with needed services

• Coordinate in-home care or nursing facility support

• Advocate for members to ensure they receive the right level of care

• Document and follow state guidelines, company policies, and case management best practices

Qualifications

• Must live in Kankakee County or Northside Chicago

• Active, unrestricted Illinois license as one of the following:

o Registered Nurse (RN)

o Licensed Clinical Social Worker (LCSW)

o Licensed Clinical Professional Counselor (LCPC)

• Valid driver’s license and reliable car

Preferred Experience

• 3–5 years of clinical experience (hospital, home health, or long-term care)

• 2–3 years of case management, care coordination, or discharge planning

• Familiarity with IL waiver services is a plus

• Comfortable working independently and from home

• Strong computer skills (Word, Excel, Outlook, PowerPoint)
