Summary:


The Intake Coordinator performs a wide range of administrative duties to include home health and hospice care referral processing, patient registration, verification of benefits, program communication to potential candidates, communication with referral sources and other tasks assigned. Responsible for the timely coordination of the referral process including the review/response/coordination of care for patients referred to Home Health or Hospice for evaluation/admission and/or transfer process of patients to the Home Health or Hospice care center.

Responsibilities:

1. Responsible for coordinating new intake referrals, gathering medical records, orders, and patient insurance information.

2. Follow up on information required for referral and admission to Home Health and Hospice services.

3. Ensure compliance with applicable coverage criteria.

4. Reviews referral for appropriate care setting and communicates with Director of Patient Care and Agency Administrators and referral source when appropriate.

5. Assists in coordination of evaluation/admissions process.

6. Gathers appropriate information to assist the clinical team in scheduling the evaluation visit.

7. Communicates pertinent patient information to the care center team via approved processes.

8. Coordinates with other departments for insurance verification and payer authorization as needed.

9. Communicates and follows up regarding pending admissions as needed or as directed by the Director of Patient Care/Agency Administrator.

10. Meets productivity expectations with quality documentation of complete referrals including all required follow up.

11. Complies with policies, procedures and regulatory mandates

12. Performs other duties as assigned.

Requirements:

Knowledge, Skills and Abilities:


Minimum high school diploma or equivalent required. Minimum two years’ experience performing clerical duties preferably in a healthcare setting. Working knowledge of referrals and insurance verification. Maintain confidentiality of patient information, compliant with HIPPA requirements. Experience in electronic medical records and general computer skills.
