Must have Pennsylvania LPN or RN license!Melmark OverviewMelmark is a nonprofit organization providing clinically-sophisticated evidence-based special education, residential, vocational, and therapeutic services for children and adults diagnosed with autism spectrum disorders, developmental and intellectual disabilities, acquired brain injuries, medical complexities, and other neurological and genetic disorders.Health Care Coordinator OverviewThe Health Care Coordinator gives direct supervision and coordination for all the medical services and health care needs of the individuals receiving services at Melmark.Schedule and Salary Information
  • Monday- Friday 8a-4:30p (flexibility required)
  • On call rotation and holidays as assigned
  • Starting salary with LPN: $68,640.00 (+) based on years of experience
  • Starting salary with RN: $89,440.00 (+) based on years of experience
Requirements
  • Registered (RN) Nurse. Licensed Practical (LPN) Nurse eligible.
  • Must hold a US Driver’s License and must be able to meet requirements to become an approved Melmark driver. Must be at least twenty-one (21) years old.
  • A minimum of two (2) year experience in related field preferred
Health Care Coordinator Responsibilities
  • Ensures proper diets for all individuals. Makes sure diets are posted. Ensures annual nutritional evaluation is completed.
  • Supervises the shift nurses. Completes evaluations for the nurses.
  • Provides comprehensive admission and discharge health care information.
  • Coordinates with the Primary Care Physician (PCP) for proper medical care and wellness of all individuals. Identifies and insures treatment of communicable diseases and reports all infectious conditions to appropriate personnel.
  • Maintains 24-hour on call Nursing and Medical responsibilities
  • Supervises and takes responsibility for all nursing tasks including medication administration (training and oversight) and all medical aspects. Provides in-services for all staff and individuals regarding health care issues.
  • Ensures all required medical paperwork is completed (90-day reviews, Utilization Reviews, reports, etc.). Provides regular reviews of medical records (appointments, lab results and current proper diagnosis).
  • Attends weekly management and IDT meetings. Also, attends ISP meetings.
