RiseWell Community Services is looking for a Full-Time Care Coordinator in our Health Homes Serving Children. This role offers remote work however would require occasional work in the field serving participants.
In this role, you will provide care coordination and health promotion services to individuals throughout Long Island. Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Assist individuals in obtaining Food Stamp and Social Security benefits. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Completes all required documentation according to standards and no later than 24 hours of service delivery.
Our Care Coordination teams help individuals with serious mental illness, chronic medical conditions, or substance abuse disorders. We connect participants with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. Our goal is to make sure the participant stays on top of appointments and receives the vital care needed for a healthy life.
QUALIFICATIONS:
Minimum of a Bachelor’s degree with at least 2 years relevant experience serving children; OR Registered Nurse with 2 years relevant experience serving children; OR Master’s Degree with 1-year relevant experience serving children. Clean, valid NYS Driver’s License. Fingerprinting, criminal record check, and all necessary clearances from the Justice Center.
