HouseCalls Home Care is a leading Licensed Home Care Services Agency (LHCSA) committed to providing compassionate, high-quality care to elderly and disabled individuals across New York. We're seeking a Bilingual Patient Care Coordinator (Spanish-speaking) to join our team in our Brooklyn office.

This role plays a vital part in helping patients receive personalized services in the comfort and dignity of their homes.

Key Responsibilities

Serve as the primary point of contact for patients and families
Develop and coordinate customized home care plans
Schedule services, follow-ups, and in-home assessments
Educate patients and caregivers on conditions, treatments, and home care routines
Monitor and document patient progress and interventions
Collaborate with healthcare providers, aides, and specialists
Ensure compliance with care protocols, agency policies, and health regulations
Provide respectful, responsive, and empathetic support at all times

Qualifications

Minimum 1 year of experience in care coordination, case management, or clinical support — specifically within home care or a closely related healthcare setting
Fluency in Spanish is required
Excellent verbal and written communication skills
Proficiency in Microsoft Office and electronic health record systems
Ability to multitask and thrive in a fast-paced environment
Strong organizational skills, empathy, and attention to detail

Compensation & Benefits

Pay: $23–$26 per hour, based on experience
