Title: Social Worker - Behavioral Health & Housing Focus

We specialize in caring for patients who are often overlooked by traditional hospice providers — including those experiencing homelessness, psychiatric conditions, or limited social support. Our team goes beyond medical care to provide dignity, compassion, and practical solutions for the most vulnerable at end of life.

Position Summary

We are seeking a bilingual Social Worker (English/Spanish) with a passion for working with underserved populations. This is a supplemental role that works alongside our licensed hospice MSW, focusing specifically on benefits advocacy, housing coordination, and psychosocial support for patients with complex behavioral and social needs.

Key Responsibilities

  • Conduct psychosocial assessments, with attention to behavioral health, trauma, and housing instability.
  • Provide crisis intervention, supportive counseling, and linkage to community resources.
  • Assist patients in navigating SSI/SSDI, IHSS, Medi-Cal, and related benefit programs.
  • Coordinate housing placements (e.g., ALW, board-and-care, recuperative care, shelter).
  • Provide education and information to patients, families, and community agencies regarding practical and environmental needs.
  • Serve as liaison between patients/families and community agencies.
  • Maintain and develop contacts with public and private agencies to expand available resources.
  • Collaborate closely with the hospice care team (nurses, physicians, chaplains) to support patient care.
  • Document interventions and contribute to interdisciplinary care planning.
  • Must be able and willing to make in-person patient visits.

Compensation & Benefits

  • $40 per hour for phone-based/benefits navigation work.
  • $85 per in-person visit for field-based support.
  • Flexible per diem schedule; mission-driven work with a team dedicated to patients who need advocacy the most.

Pay: $40.00 per hour
