PURPOSE:
The Nurse Care Coordinator (NCC) Support will act as a liaison between members, caregivers, dependency case managers, providers and the NCCs. The NCC support will play a crucial role in helping to identify when Nurse Care Coordination is needed and will communicate the members’ needs to the assigned NCC. Education will be provided to members, caregivers and dependency case managers about health care benefits and resources, including after-hours supports.
KEY RESPONSIBILITIES:
- Process record requests assigned by Nurse Care Coordinator.
- Process incoming records for Nurse Care Coordinators to review.
- Assist members/caregivers with finding in-network dental and healthcare providers as needed.
- Provide education regarding access to health care benefits, including expanded benefits, emergency room alternatives and after-hours resources.
- Provide education on routine well exam and dental visits.
- Assist with Child Welfare Health Plan primary care provider changes as needed.
- Assist with applications for available grants as needed.
- Complete post hospital visit screening with member, caregiver or dependency case manager as assigned and notify the Medical Team and/or Behavioral Healthcare team when care coordination needs are identified.
- Complete screening with member, caregiver or dependency case manager when child/youth changes placements as assigned and notify the Medical Team and/or Behavioral Healthcare team when care coordination needs are identified.
- Participate in child specific staffing meetings as assigned and notify the Medical Team and/or Behavioral Healthcare team when care coordination needs are identified.
- Notify the assigned NCC of any pending medical needs or when barriers accessing healthcare benefits are identified.
- Communicate upcoming appointment information obtained from members, caregivers and dependency case managers to the Medical Team.
- Assist with obtaining vaccine records as needed.
- Assist caregiver/CBC when bills received for services; communicate with provider to help meet resolution.
- Document all NCC support duties into the Child Welfare Health Plan’s designated location by the next business day, or FSFN if the member is not assigned to the Child Welfare Health Plan.
- Other duties related to supporting the Medical Team as assigned.
EDUCATION / EXPERIENCE:
- High School Diploma/GED. College courses a plus.
- Three (3) years’ experience in an administrative/clerical support position preferred.
- Experience in a not-for-profit organization a plus.
