**This is an IN OFFICE ADMINISTRATIVE position**
Cranberry Home Care is seeking a highly motivated and skilled individual with strong organization and time management skills the opportunity to work with this growing team as our Client Care Manager! This individual will develop strong relationships with caregivers and clients to ensure the care being provided to our clients meets our company’s core values. This position is part time, 25 hours a week (daytime hours) with schedule flexibility. Position is primarily in office, with some travel as needed.
This in-office position is located at 43 E. Grove Street, Middleboro, Ma.
The hours are Monday – Friday 9:00AM – 2:00PM (25 hours.)
Local travel requirements ~ 20%. Mileage reimbursement provided.
Duties & Responsibilities may include but are not limited to:
- Manage and oversee client case load.
- Responsible for meeting with clients and their family and performing initial client care & safety assessments.
- Evaluate client’s needs and goals and utilize the WellSky platform to develop an individualized care plan and tasks for each client. Conduct on-going client assessments to ensure care needs and goals are being met.
- Responsible for completing the first shift with each individual client to ensure care plan is properly aligned with the level of care needing to be provided.
- Review field documentation daily to ensure tasks are completed as written in the care plan. Monitor documentation for any changes in condition that would require an update to the care plan; conduct follow-up calls/visits as needed.
- Oversee the day-to-day client care needs by supervising, directing, and coaching field staff.
- Complete check-in calls with all field staff to discuss client care needs, questions or concerns that may arise throughout the week.
- Act as the primary point of contact between clients and their family members. Respectfully address concerns and help develop, implement, and adjust client care plans as needed.
- Attend team meetings as scheduled.
- Educate and train caregivers on certain skills or tasks.
- After hours “on-call” responsibility, this includes triaging Answering Service messages and following up. This may include responding to questions or needs of caregivers and/or clients, addressing callouts by trying to find coverage. You would NOT be responsible to cover/work a shift after hours. Days assigned TBD.

Education and Experience:
- Minimum 2 years hands-on experience working in geriatrics. This requires knowledge in performing ADLs on a variety of individuals, and use of common medical equipment. Ability to perform and teach transfers skills (gait belt, mechanical lifts, stand/pivot etc.). REQUIRED
- Prior home care experience. PREFERRED
- At least 1 year healthcare management experience (managing staff & clients). REQUIRED
- Valid Massachusetts driver’s license and automobile insurance. REQUIRED
Physical Requirements
- Prolonged periods of driving from one client to another
- Ability to sit at a desk and work on a computer
- Must be able to lift up to 50lbs at a time
Job Type: Part-time
