Social Worker (Full-time)

About the Role

CareOne at Middletown is seeking a compassionate and dedicated Full-Time Social Worker to join our interdisciplinary care team. In this role, you’ll provide emotional support, care planning, and discharge coordination for residents and their families, helping them navigate the challenges of long-term care with dignity and respect.

Key Responsibilities

• Participate in interdisciplinary care planning to support residents’ psychosocial well-being
• Provide therapeutic support to help residents cope with illness, disability, and institutionalization
• Conduct initial psychosocial assessments and maintain ongoing documentation per company policy
• Develop and update the social services component of each resident’s care plan
• Facilitate family meetings, support groups, and discharge planning
• Act as a liaison between residents, families, outside agencies, and facility leadership
• Assist with admissions coordination, including referrals, insurance verification, and facility tours

Qualifications

• Education: Master’s Degree in Social Work or a related human services field
• Licensure: CSW or LSW in the state of New Jersey (required)
• Experience:
• Minimum 1 year of social work experience
• Long-term care or skilled nursing facility experience preferred
• Skills:
• Strong communication and documentation skills
• Ability to work collaboratively in a team-based environment
• Empathy, professionalism, and a resident-first mindset
