POSITION SUMMARY
The Care Manager Health Plex, provides onsite patient referral services for specialty clinics, private physician, pain management and behavioral health. This position assists patients with the pharmacy assistance program, applications for medical assistance/welfare, disability, MED 1 forms and any other relevant applications necessary for patient care management. In addition, this position assists with managing and completing the LOA Medicaid referrals, follows up on eligibility and performs the monthly audits for LOA compliancy.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS
EDUCATION: High School Diploma required; Bachelor's degree social services preferred.
CERTIFICATION/LICENSE: Current New Jersey Driver's License required.
EXPERIENCE: Minimum of 2 year experience in community based social services required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.
WORK ENVIRONMENT
The potential for exposure to the hazards and risk of the ambulatory environment, including exposure to infectious disease, hazardous substances, and potential injury. This position requires sitting, standing and walking a majority of the workday. Works with such equipment as computer terminal, fax machine, printer and copier. The essential functions for this position are listed on the Assessment and Evaluation Tool.

This position reports to department leadership.
