SUMMARY OF JOB:

Responsible for assessing patient/family psychosocial needs and coordinating individualized psychosocial plan of care in the home, nursing home, or inpatient hospice setting. Provides psychosocial support, counseling, and resource planning in accordance with the plan of care. Serves as a resource for the hospice team, organization, and community regarding psychosocial aspects of end of life care. Participates in professional development activities that further personal and program goals related to end of life care.

RECRUITMENT REQUIREMENTS:

Master’s degree from an accredited school of Social Work; Master’s degree in Psychology, Sociology or other field related to social work is acceptable with supervision by a licensed MSW, in accordance with ACHC requirements.
Social Work License preferred
Minimum one to two years post-graduate experience in a health care setting preferred
Minimum one year working with terminally ill patients preferred
Ability to work within an interdisciplinary team

LICENSE AND REGULATORY REQUIREMENTS:

Current license in state of practice
Valid driver’s license
