Hospice Master Social Worker
Harmony Hospice is seeking a compassionate and caring Hospice Social Worker to join our team. Our dynamic culture separates us from our competitors. Our service area includes Manistee County, Mason County, and Oceana County.
Key Responsibilities:
- Provides individual and family counseling, and crisis intervention
- Evaluates coping skills in crisis situations
- Fosters communication between patient and families
- Assists patient and family in acquiring additional community resources, including Medicaid; serves as liaison with social service agencies as necessary
- Participates in program and policy development of social work component
- Documents patient changes appropriately
Position Requirements:
-Licensed from an accredited School of Social Work
- 2 years’ experience in a health-related area
- MSW, valid driver’s license and vehicle, CPR certification
- Ability to travel within the service area
Job Type: PRN to start
Pay: Based on experience and is negotiable
