Job Summary
The Care Coordinator plays a vital role in ensuring that individuals receive the appropriate care and support they need. This position involves working closely with residents, families, and healthcare providers to develop and implement personalized care plans. The ideal candidate will have experience in various care settings, including assisted living, adult care homes, and/or memory care facilities, and will demonstrate a strong commitment to enhancing the quality of life for those they serve.

Duties

  • Collaborate with healthcare professionals to create and manage individualized care plans for clients.
  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, and meal preparation.
  • Provide direct support to individuals with developmental disabilities, dementia, and Alzheimer’s disease.
  • Monitor patient progress and adjust care plans as necessary.
  • Maintain accurate documentation using EMR systems while adhering to HIPAA regulations.
  • Conduct vital signs checks and assist with medication administration as needed.
  • Facilitate communication between clients, families, and healthcare providers to ensure continuity of care.
  • Engage in conflict management and behavior management strategies when necessary.
  • Participate in case management activities to coordinate services and resources for clients.
  • Provide training and support to caregivers regarding best practices in patient care.

Skills

  • Experience in assisted living, adult foster care settings, or senior care environments.
  • Knowledge of medical terminology and first aid procedures.
  • Familiarity with cleaning protocols in caregiving settings.
  • Ability to perform heavy lifting when assisting clients with mobility needs (e.g., using a Hoyer lift).
  • Proficient in working with individuals with disabilities, including autism.
  • Strong organizational skills for managing multiple cases effectively.
  • Excellent communication skills for interacting with clients, families, and multidisciplinary teams.
  • Compassionate approach to caregiving that prioritizes the dignity and respect of all individuals.
  • Ability to drive as required for client transportation or community outings.
