Position Summary
Under the direction of the Program Director and working as part of an interdisciplinary team, the ECM Program Manager is responsible for the daily operations of the Community Bridges ECM program. The program addresses service gaps related to poor health outcomes for Medi-Cal recipients. Responsibilities include direct care management, program development, implementation, quality improvement, staff training, supervision, coordination, motivation, and related tasks.
