Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and advocating for patients' needs throughout their treatment journey. This role requires a strong background in various medical fields, including critical care, emergency medicine, and behavioral health, to provide comprehensive support to patients in both inpatient and outpatient settings.
Duties
- Conduct thorough patient assessments to determine individual needs and develop personalized care plans.
- Coordinate discharge planning and ensure smooth transitions for patients from acute care to post-acute or outpatient services.
- Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to facilitate optimal patient outcomes.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide education and resources to patients and families regarding treatment options and available services.
- Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
- Engage in utilization management to ensure appropriate resource allocation and effective care delivery.
- Advocate for patients' rights and needs within the healthcare system.
Qualifications
- Critical care experience is preferred, with familiarity in Level I or Level II trauma center settings.
- ICU experience is highly valued; candidates with backgrounds in hospital medicine or emergency medicine will be prioritized.
- Knowledge of infectious disease care, geriatrics, behavioral health, pediatrics, or medical-surgical nursing is advantageous.
- Experience in case management, discharge planning, intake processes, and post-acute care is essential.
- Strong understanding of medical terminology, physiology knowledge, vital signs monitoring, and diagnostic evaluation techniques.
- Proficiency in managing medical records while adhering to HIPAA guidelines is required.
- Previous experience in outpatient clinics or primary care settings is a plus.
