This position is primarily in the office, but is expected to be willing and capable to cover in the field as needed.

The Referral / Care Coordinator will be responsible to:

· assist with all agency communication between clients, Caregivers, family members, and other professionals

· must have competent computer skills

· pay close attention to detail

· possess the ability to manage confidential information

· have good phone etiquette

· able to remain professional at all times

Work hours: Monday - Friday 8am-5pm with some "on-call" required.

Daily Responsibilities will Include:

• Scheduling – Assist scheduler
• Data Entry
• Referral Processing
• Customer Service Skills
• Working on Business Development with referral sources

Job Requirement: Applicants must have a valid driver’s license, Insurance, and a reliable vehicle. Must have US work authorization (Required)

Job Type: Full-time

Pay: $15.00 - $18.00 per hour depending on experience and level of education/degree
