Position Summary

We are seeking a Licensed Master Social Worker (MSW) to join our interdisciplinary hospice team. The ideal candidate will be compassionate, motivated, and skilled in providing emotional, psychosocial, and resource-based support to patients and families navigating end-of-life care.

The MSW plays a vital role in supporting patients’ quality of life, advocating for their needs, and ensuring families receive the guidance and resources they need throughout the hospice journey.

Responsibilities

  • Provide psychosocial assessments and ongoing support to hospice patients and their families.
  • Offer counseling, crisis intervention, and grief support tailored to individual and family needs.
  • Collaborate with the interdisciplinary team (nurses, aides, chaplains, physicians) to create and implement individualized care plans.
  • Connect families with community resources, financial assistance, and supportive services.
  • Document all interactions in compliance with hospice and Medicare guidelines.
  • Participate in team meetings, care conferences, and in-service trainings.

Qualifications

  • Master’s Degree in Social Work (MSW) from an accredited program.
  • Current state licensure as an MSW.
  • Previous hospice, palliative care, or healthcare experience preferred.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently while contributing to a team environment.
  • Compassionate, empathetic, and committed to supporting patients and families.
