Job Overview
We are a PHP/IOP in La Jolla, CA seeking a dedicated and experienced Substance Abuse Counselor/ Case Manager to join our team. The ideal candidate will have a strong background in addiction and mental health counseling and support, with the ability to provide effective therapeutic interventions for individuals struggling with substance use disorders. This role requires a commitment to helping clients achieve recovery through evidence-based practices and personalized care plans.
Responsibilities
- Client Coordination: Assist clients in obtaining services needed for medical, housing, education, and other.
- Group Facilitation: Lead group sessions for clients with substance use disorders, using evidence-based techniques and therapeutic modalities to promote recovery and personal growth.
- Aftercare Planning: Help clients develop an aftercare plan including a supportive environment and support system network for client's discharge.
- Conduct Assessments: Perform periodic assessments to evaluate client's needs to be able to provide individualized support.
- Collaboration: Collaborate with the treatment team, including therapists, counselors, and other case managers, to ensure coordinated and holistic care.
- Conduct comprehensive assessments and intake evaluations for clients seeking substance abuse treatment.
- Develop and implement individualized treatment plans based on client needs and goals.
- Provide individual and group counseling sessions utilizing various therapeutic techniques, including cognitive behavioral therapy and motivational interviewing.
- Facilitate crisis intervention and management strategies to support clients in acute situations.
- Collaborate with multidisciplinary teams to coordinate care, ensuring holistic support for clients.
- Maintain accurate medical documentation, including patient records, treatment progress notes, and compliance with HIPAA regulations.
Skills
- Proficient in behavioral therapy and crisis management.
- Experience in Adult counseling, PTSD care, and addiction medicine is preferred.
- Strong understanding of medical terminology and medical documentation practices.
- Ability to conduct diagnostic evaluations and case management effectively.
- Knowledge of group therapy dynamics and individual counseling strategies tailored to diverse populations.
- Familiarity with applied behavior analysis principles and behavior management techniques.
- Excellent communication skills with a focus on patient care and assessment.
Qualifications
- Strong knowledge of therapeutic techniques and recovery principles.
- Excellent communication and interpersonal skills.
- Empathetic and non-judgmental approach to group facilitation.
- Ability to work both independently and as part of a team.
- High School Diploma or equivalent
- CADC I or CADC II
- Experience working with adults and adolescents with substance use and co-occurring disorders is preferred
- Knowledge and experience using KIPU EMR
- Availability to work during program hours: 9am - 3pm Monday through Friday
