About Job

Essential Duties

  • Provides direct case management services where the Member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services.
  • Conducts a comprehensive assessment and gathers all information pertinent to the case to assist with developing an individual care plan. Develops, implements, and monitors an individual care plan in conjunction with the Member, family, and multidisciplinary team to achieve the desired outcomes.
  • Monitors established measurable goals and routinely assesses the Member's status and progress in order to proactively make appropriate recommendations for adjustments in the individual care plan and/or services to promote better outcomes.
  • Establishes and maintains effective communication and collaboration with Member's multidisciplinary treatment team regarding Member's status, services, and progress. This includes scheduling and facilitating treatment planning meetings with Member, family, and multidisciplinary treatment team.
  • Provides linkage and assistance to Member in applying for and connecting with public and community benefits and resources, including educating Member on qualifying criteria for various benefits and resources.
  • Advocates for and provides support to Member with healthcare professionals, which may include attending appointments with Member.
  • Complies with productivity standards of the program and maintains an appropriate caseload to reach the budgeted daily expectation of the number of billable client hours/contacts. May be assigned to multiple program cases among more than one program based on need.
  • Submits timely documentation in compliance with contract and agency expectations according to caseload notes.
  • Accurately completes all required documentation for assigned Members as required by contract and Agency within the agency's Electronic Health Record system and as measured by QA standards and audit results.
  • Participates in supervision by coming prepared to discuss current caseload, documentation, and productivity; accepts constructive feedback and direction; and puts feedback into therapeutic practice with Members.
  • Maintains confidentiality of all information regarding Members, families, and operations of the Agency (including media).
  • Maintains a professional relationship with Members, staff, and other outside stakeholders at all times.
  • Exercises good judgment in the performance of duties and responsibilities.

Qualifications

  • High School Diploma or GED required. Bachelor's Degree in social service field preferred.
  • Previous experience with providing case management services and/or care coordination for vulnerable and /or underserved and diverse populations, preferred.
  • Availability of a personal car with appropriate insurance which employee is willing to use for all Agency business if needed; adhering to federal motor vehicle safety standards when transporting Members.
  • Effective oral/written communication skills; bi-lingual skills in Spanish/English preferred.
  • Computer skills sufficient to meet production demands of the job and skill in maintaining detailed, accurate and complete records.
  • Must have a valid California Driver License, proof of automobile insurance, and meet the Agency's safe driver guidelines.
  • Ability to work varied hours including evenings, nights, and weekends, if needed.

Compensation Range: $22.50/hr - $30.98/hr
