Pay: $24.93 – $33.59/hr (based on experience; midpoint $30.54 at ~10 yrs)
Sign-On Bonus: Not advertised
Relocation Assistance: Case-by-case
Schedule: Days with rotating weekends (typical 8:00–4:30; may be 9:00–5:30 or 9:30–6:00)


Job Description


Overview

A 260+ bed acute care hospital in the Fort Walton Beach, FL area is hiring a Social Worker (LCSW) to provide non-clinical, biopsychosocial assessments and interventions for inpatient units and the Emergency Department. Scope includes crisis intervention, high-risk screening, focused emotional support, resource referrals, and post-acute discharge planning across ICU, Trauma, Oncology, NICU, Transplant, and ED. Reports to the Director of Case Management.

Responsibilities

  • Complete thorough biopsychosocial assessments; identify barriers to discharge (financial, emotional, psychosocial, medical, spiritual).

  • Lead discharge planning for high-risk patients; coordinate post-acute placements and services.

  • Provide crisis intervention, counseling, education, advocacy, and resource linkage for patients/families/caregivers.

  • Facilitate family/team conferences; support decision-making aligned with patient values.

  • Maintain working knowledge of Medicare/Medicaid/private payer rules, managed-care concepts, LOS management.

  • Collaborate closely with RN case managers, nurses, physicians, payers, and community partners.

  • Support patients/families through complex and emotionally charged situations (end-of-life, terminal illness notifications, death notification with physician).

  • Respond to social work consults across the facility as needed.


Qualifications


Required

  • Active Florida LCSW (required).

  • Master of Social Work (MSW) (required).

  • Recent acute care hospital Social Worker experience with discharge planning (within last 3–4 years).

  • Experience with PASSRs and ESPERs.

Considered

  • Candidates not in hospital setting within last 5 years may be considered if they have 5 years prior hospital Social Worker experience.

  • LTAC or Hospice Social Worker experience plus some hospital experience may be considered.

Not a fit

  • Candidates with only psych or only community therapist backgrounds (no hospital discharge planning).
