Job Overview - LPN Nurse Care Manager (Foster Care & Youth Services):
Compensation: $40/hour
Location: Bronx, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Provide nursing and care coordination as an LPN Nurse Care Manager with our client in the Bronx, NY. You will assess adolescent health needs, coordinate medical homes, administer medications, and provide health education while fostering independence. Act as a liaison between youth, families, and providers, ensure compliance in documentation, and apply trauma-informed care practices. Responsibilities as the LPN Nurse Care Manager:
- Health Assessments & Care Planning: Conduct health assessments, home visits, and develop service plans to address the needs of adolescents and caretakers.
- Care Coordination: Serve as the liaison among providers, families, and multidisciplinary teams while establishing medical homes to ensure continuity of care.
- Medication Management: Administer prescribed medications, monitor effectiveness, and maintain documentation in compliance with regulatory standards.
- Client Education & Support: Educate youth and families on conditions, medications, and self-care practices while fostering independence and providing support.
- Trauma-Informed Care: Implement Sanctuary Model practices to create a safe, non-violent, and trauma-sensitive environment for youth in care.
- Education: Graduate of an accredited practical nursing program required.
- Experience: 2+ years of nursing experience in pediatrics, adolescent health, foster care, or youth services, with trauma-informed care experience preferred.
- Licensure & Certification: Active New York State LPN license and current CPR certification (American Heart Association or American Red Cross) required.
- Technical Skills: Strong knowledge of nursing principles and practices, with the ability to manage medical records and coordinate across multidisciplinary teams.
- Skills & Attributes: Strong communication, compassionate care, cultural sensitivity, and collaboration with youth, families, and staff, including support at medical appointments.
