About Job

Overview:
JOB SUMMARY
Manage a daily caseload of patients that need a variety of social service interventions. Conduct psycho-social assessments and make recommendations for post hospital care on patients with mental health concerns. Work collaboratively with the case managers in the discharge planning process. Assist patient and their family and/or support systems with coping skills as it relates to major medical illnesses.

MINIMUM QUALIFICATIONS

Education:
A Masters degree (MSW), from an accredited School of Social Work.

Experience:
One year acute hospital experience or medical social work experience preferred.

Knowledge and Skills:
Knowledge of mental health disorders symptoms and treatment. Ability to comply with mandatory reporting requirements, ability to evaluate/assess patients with psychiatric disorders and develop safe discharge plans. Knowledge of the relationship between medical illness and behavioral health problems related to short term interventions and solutions. Ability to conduct a variety of social work assessments and interventions. Knowledge of community resources.

License/Certifications:
Current American Heart Association (AHA) BLS card.

Equipment:
Computer literate.

Physical Requirements:
Must be able to perform essential physical job requirements.

PAY RANGE
$29.96 - $44.94

The posted pay range reflects the lowest to highest pay that was available for this position at the time of posting and may be subject to change. Salary offers are determined by candidate’s relevant experience and skills. For per diem positions, a standard rate is used based on market data and not the candidate’s individual experience.
