The Complex Care Manager provides time-limited interventions to restore functioning and address the symptoms of mental illness, including skill-building to help individuals identify solutions to problems that threaten recovery and care coordination services to help individuals connect with medical or other remedial services. This service will be provided to address immediate mental health issues or factors that are impacting on the individual’s health or community status. Responsibilities of this position are determined by needs of the Mental Health Program under OMH guidelines/requirements. As part of the essential functions of this role, the Complex Care Manager:
- Conduct screenings/assessments to identify an individual’s clinical and social determinants of health (SDoH) needs, choices, and their preferences for services/providers.
- Assess and address health and safety issues as well as barriers to care and barriers to patient’s ability to remain in a community setting.
- Maintain communication between treating practitioners and other service providers to facilitate collaborative care for complex behavioral health needs.
- Provide additional referral options as appropriate (i.e. primary care, testing, substance use counseling, case management, housing information, social services).
- Provide ancillary services to patients that have a high rate of hospital utilization to decrease readmission and emergency room occurrences.
- Assist in collaboration among cross-system agencies, organizations, and other entities to facilitate an organized and coordinated approach to care.
- Provide education and referral/linkage to needed support and services to meet the comprehensive needs of individuals/families.
- Participate in individual and/or group supervision; coordinate with treatment team as appropriate to provide comprehensive services.
- Provide outreach to collateral contacts by telephone with patient’s family members and/or treatment providers.
- Facilitate referrals for patient care to providers inside and outside of Evergreen Health, as requested by a member of the CBH treatment team.
- Facilitate data collection and report generation related to patient care, as requested.
- Obtained medical records from external and internal providers as requested by patients care team.
- Advocate on behalf of the individual
- Promote self-advocacy and the ability to self-direct
- Complete required documentation into electronic charting system in a timely manner.
- Identify and develop relationships with community agencies that can assist with promoting programs and services targeted to high-risk communities.
- Provide a positive, welcoming, harm reduction, trauma informed, and customer service-oriented experience for patients.
- Handle Protected Health Information (PHI) in accordance with federal and state privacy laws, including the Health Insurance Portability and Accountability Act of 1996.
- Attend all meetings, training events, functions, special events, in-services or educational programs as required by Evergreen Health. Maintains appropriate communication and interpersonal skills with supervisors, staff, and patients to ensure coordinated efforts and provision of high-quality service.
Qualified Candidate will have a bachelor’s degree in psychology, social work or other related field with two (2) years of experience in human services or behavioral health setting – OR – a Masters degree with one (1) year of experience. Candidate will have a familiarity with electronic charting systems (Medent preferred). Candidate must be able to communicate clearly and professionally, and demonstrate personal responsibility and integrity. Ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA a must. Sensitivity to and experience with cultural diversity, Harm Reduction, LGBTQ population, Substance Use population and people living with HIV/AIDS issues is essential.
Job Type: Part-Time (20 - 39 hours per week)
Required education/experience: Bachelor’s degree in psychology, social work or other related field with two (2) years of experience in human services or behavioral health setting – OR – a Master’s degree with one (1) year of experience
