Job Description


Job Type: Full-time (40 hrs per week)

   

Benefits: Salary: up to $80,000 based on experience

 Paid Time Off (PTO): 2 weeks (equivalent to 80 hrs)

Sick Leave: 1 week (equivalent to 40 hrs- rollover allowed)

Reports to: Medical Director


Job Purpose:

  • To advocate and provide case management and support for LifeSpring families.  
  • Liaise between the general pediatric and developmental and behavioral health departments.
  • LifeSpring demonstrates the love of Jesus Christ by providing exemplary pediatric, community, and behavioral healthcare for the families we serve.


Skills and Requirements: 

  • MS degree in social work (MSW) and licensed (LCSW)
  • 3 years of social work experience, pediatric experience preferred
  • Working knowledge of social theories and practices
  • Ability to work under pressure, multi-task, and change priorities quickly
  • Excellent verbal and written skills
  • Professional computer skills 
  • Attentive to detail and highly organized
  • Social perceptiveness and empathy
  • Ability to build and maintain professional helping relationships
  • Ability to relate and communicate with diverse populations and groups
  • Spanish proficiency preferred

Tasks and Responsibilities:

  • Embody the love of Christ in all interactions with co-workers and patients.
  • Introduce patient families to LifeSpring developmental and behavioral health services and facilitate their transition between all departments as indicated
  • Conduct psychosocial assessments for children and their families to identify emotional, social, and environmental needs
  • Provide counseling and therapeutic interventions to address adjustment to illness, trauma, and distress related to medical or psychosocial conditions
  • Deliver crisis intervention as needed and coordinate with law enforcement and legal partners when indicated
  • Connect families with appropriate community and financial resources to address non-medical needs impacting care 
  • Educate families about illness management, treatment plans, coping strategies, and stress reduction.
  • Participate in interdisciplinary team meetings, case consultations, and continuous quality improvement
  • Maintain accurate, confidential patient records in compliance with clinic, state, and federal guidelines.
  • Maintain LCSW licensure and associated continuing education
  • Maintain confidentiality and adhere to HIPAA, OSHA, and professional ethics standards at all times.
  • Other clinical duties as assigned
