RESPONSIBILITIES: Assess and offer psycho-social, and or substance use service intervention/treatment to vulnerable patients. Provides patient centered medical case management services, health education and wellness services. Coordinate referrals to services available through the organization and other agencies. Actively participate in performance improvement and peer review process. Serve as an integral member of a multi-disciplinary care team.
DUTIES:
Provide care coordination along with primary care team to assist patients with chronic medical mental health conditions and substance use disorders.
1. Conduct substance use and mental health assessments, utilizing standardized assessment tools and best practice techniques.
2. Educate patients and link them to appropriate agencies, i.e. detoxification and rehabilitation programs, long-term residential programs and Article 28 primary care.
3. Support and motivate patients to achieve their health goals by coaching them through behavior change and identifying their strengths and community support systems.
4. Provide patients with crisis counseling and advocacy, as required.
5. Demonstrate the ability to provide treatment that is recovery oriented, supports evidence based practice.
6. Make psychosocial assessments utilizing all available data, including interviews with patients, spouses or family members if applicable.
7. Maintains a caseload as required.
8. Timely and accurate record keeping including treatment planning, progress notes and required correspondence for patient care documenting each patient encounter in the electronic medical record (eCW) in accordance with established policies and procedures.
9. Supervise social work students in their fieldwork placement.
10. Ensure all cases of suspected child abuse or neglect are reported to the New York State Central Registry.
11. Assess the ability of patients to manage their chronic disease(s) and work with the primary care team to connect them to appropriate education and monitoring programs.
12. Attend regular team and departmental meetings to discuss patient progress and update care plans; communicate with patients' primary care providers.
13. Participate in staff committees, including but not limited to meetings of the Social Services Department and Performance Improvement Committee.
14. Participate in internal and external meetings and outreach activities.
15. Adhere to the organizations policies and procedures and clinical protocols, confidentiality and other standards of professional conduct. Participate in continuing education to maintain a solid knowledge base through in-service, and through other forms of continuing education.
QUALIFICATIONS:
Master's degree with two-four years of supervisory experience in the following fields of social work, mental health or psychology
At least two years of experience working in human services with individuals with serious mental health illnesses and co-occurring substance use disorders
Experience with various evidence-based models and theories pe1iaining to psychoeducation, motivational interviewing, serious mental illness and recovery.
Experience understanding family systems based on cultural and ethnic backgrounds Highly organized and self-motivated
Bilingual Spanish strongly preferred.
Experience working with patients and families, including individuals with HIV/AIDS, a history of substance abuse, mental illness, domestic violence and families at risk.
Dedicated to providing high quality patient-centered services.
Active New York State as Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Psychologist (PhD/PsyD)
Job Type: Full-time
Pay: $80,000.00 - $90,000.00 per year
