The Support Service Coordinator plays a critical role in connecting youth and young adults to resources, housing, and supportive services. This position will be responsible for intake, assessment, case management, housing navigation, and outreach activities, while also supporting the administrative needs of Tiny’s Village. The ideal candidate is highly motivated, compassionate, and experienced in working with individuals impacted by homelessness and systemic barriers.
Key Responsibilities
- Conduct intakes and assessments to determine participant needs.
- Provide case management services and create individualized service plans.
- Assist youth with housing navigation, including applications, referrals, and housing retention support.
- Conduct street and community outreach to identify and engage eligible participants.
- Support day-to-day office operations, including data entry, scheduling, and reporting.
- Build and maintain strong, supportive relationships with participants.
- Attend team meetings, staff trainings, and community partner meetings.
- Maintain accurate, timely case notes and reporting in data systems.
- Collaborate with community partners, agencies, and service providers to connect participants with resources.
Qualifications
- Minimum of 2 years of experience in social services, housing services, or homeless services.
- Knowledge of local housing resources, systems, and referral processes.
- Strong organizational, administrative, and data-entry skills.
- Excellent interpersonal and communication skills, with the ability to build trust with youth and young adults.
- Commitment to trauma-informed care and harm reduction practices.
- Ability to work independently and as part of a team.
- Must be able to attend community events and perform outreach in the field.
Preferred Qualifications
- Experience with HMIS or other case management databases.
- Bilingual (Spanish/English) preferred.
- Knowledge of Los Angeles County homeless service systems (LAHSA, Coordinated Entry System, etc.).
Job Type: Part-time
Pay: $21.00 - $25.00 per hour
Expected hours: No more than 30 per week
