Overview
The Home Health Community Liaison plays a key role in expanding awareness of CCHS throughout Metro Atlanta. In this part-time position, you will connect with referral sources, cultivate community partnerships, and represent our agency with professionalism and care. Success in this role means driving growth while staying true to our mission of helping individuals live safely and comfortably at home.

Responsibilities

  • Build and maintain strong relationships with key community organizations.
  • Represent CCHS at community events and identify new opportunities to promote services.
  • Educate the community on our home health services and the benefits offered by the agency.
  • Follow up on leads and support conversion to active, paying clients.
  • Work with the home office to plan marketing initiatives and provide feedback to support company growth.
  • Create and maintain proper documentation, records, and reports of outreach activities.
  • Stay informed on industry trends, and new opportunities while advocating for the agency in the community.
  • Maintain a professional presence that reflects the agency’s values of compassion, integrity, and person-centered care.

Qualifications

Required:

  • High school diploma or equivalent.
  • 1+ year of experience in healthcare, marketing, sales, or community outreach.
  • Strong communication and interpersonal skills with the ability to build trust quickly.
  • Comfortable with regular travel across Metro Atlanta for community outreach and events.
  • A current driver’s license and reliable transportation.
  • Ability to organize, document, and report outreach activities clearly and accurately.
  • Self-motivated, reliable, and able to work independently with minimal supervision.

Preferred:

  • Bachelor’s degree in a relevant field (marketing, communications, healthcare, or related).
  • Previous experience in home health, senior care, or healthcare marketing.
  • Established connections with local referral sources
  • Familiarity with Waiver programs, Veteran’s Affairs’ benefits, and private pay services.
  • Experience supporting social media or digital marketing campaigns.
  • Strong understanding of local healthcare trends and community resources.

Job Types: Part-time, Temporary

Pay: $40,945.57 - $60,149.51 per year

Expected hours: 15 – 20 per week
