Position Overview

We are seeking a Community Outreach & Care Coordinator to lead our local marketing, referral partner engagement, and client onboarding efforts. This role is ideal for a relationship-driven professional who thrives on connecting with the community, building trust, and helping families access the care they need.

You will be responsible for creating and attending community events, nurturing referral partnerships, following up on leads, conducting in-home care assessments, and ensuring ongoing family satisfaction through quarterly visits.

Key Responsibilities

Plan, coordinate, and attend community events to promote our services.

Build and maintain relationships with referral partners, including healthcare providers, senior centers, and community organizations.

Respond promptly to incoming leads and inquiries.

Conduct in-home care assessments to understand family needs and recommend services.

Serve as the main point of contact for new clients, ensuring a smooth onboarding process.

Visit families quarterly to ensure satisfaction and identify additional support needs.

Maintain accurate records of outreach, leads, and client engagement activities.

Qualifications

Proven experience in community outreach, sales, marketing, or client relations (healthcare or senior services preferred).

Strong interpersonal and communication skills.

Self-motivated, organized, and able to work independently.

Knowledge of the local senior care landscape is a plus.

Valid driver’s license and reliable transportation required.

Compensation & Benefits

Base Salary: $45,000 – $50,000/year (based on experience)

Bonus: 3% ongoing commission on all revenue generated from families you originate, for the lifetime of the client relationship.

Mileage reimbursement.
