Position Summary: Adult Rehabilitative Mental Health Worker

As an Adult Rehabilitative Mental Health Worker, you will play a crucial role in supporting individuals with mental health challenges on their path to recovery and wellness. Responsibilities include conducting assessments, developing individualized treatment plans, and implementing interventions that focus on skill-building and promoting independence. You will collaborate with a multidisciplinary team, engage clients in therapeutic activities, and provide support to enhance their daily living skills. The goal is to empower clients to achieve their mental health goals and improve their overall quality of life through a person-centered and strengths-based approach. This position will also include Housing Stabilization Services tasks.

Position Summary: Housing Stabilization Services Worker

As a Housing Stabilization Services Worker, your primary responsibility is to assist individuals, particularly those facing housing instability or homelessness, in securing and maintaining stable housing. This role involves conducting needs assessments, developing housing plans, and connecting clients with available resources and support services. You will collaborate with landlords, property managers, and community organizations to identify suitable housing options and negotiate leases. Additionally, you will provide ongoing support to clients in addressing challenges related to housing stability, such as financial management and community integration. The aim is to facilitate long-term housing stability and promote the overall well-being of individuals in need.

OVERVIEW:

The ideal candidate for this opportunity will:

- meet State of Minnesota requirements for a Housing Navigator.

- meet State of Minnesota requirements for a Mental Health Practitioner

-have knowledge of housing supports.

- must pass a Department of Human Services background study and

sexual exploitation screening

-have a valid driver’s license and acceptable driving record.

-be at least 18 years of age.

Essential Duties and Responsibilities

  • Provide crisis assessment and intervention to someone in a mental health crisis
  • Provide housing searches, advocacy, and support to locate, sustain or transition to housing

•Teach coping skills and home-based skill training

  • Help problem solve
  • Provide empathetic listening and validation
  • Coordinate services with other programs and providers
  • Provide culturally responsive mental health services to recipients and/or their families in the

home or community, based on the treatment plan

  • Assist recipients in identifying goals for functional improvement and work with them to meet

their goals

  • Design and implement treatment/intervention plans and functional assessments in a timely

manner

  • Assist with collecting assessment data through interviews and 1:1 interactions

•Provide mental health services in-home and in the community

  • Observe physical, psychological and safety status of recipients on an ongoing basis

•Assess an individual’s potential for self-injury or injury to others

  • Report observed changes and changes reported by recipient
  • Maintain client confidentiality and must comply with HIPAA laws
  • Ensure all documentations are completed in a timely manner (treatment plan, functional

assessment, progress notes, etc.) in the electronic health record system (EHR).

  • Session/intervention documentation and billable time entry into the electronic health record

system

  • Attend individual, group supervisor and staff meetings/training
  • Maintain consistent Continuing Education and Training as required (CEUs)
  • Maintain minimum billable hours weekly
  • Outreach to other area providers and community to promote programming

COMPENSATION:

This is a rewarding opportunity offering an hourly pay of $19.00 to $22.00 per hour, based on years of

experience.
